WHAT TO EXPECT DURING A PROFESSIONAL CONSULTATION VISIT
Access Options’ professional consultation services include caregiver support and education as needed. Our Occupational Therapists will work with caregivers to provide information about medical conditions such as dementia, respiratory diseases, and Parkinson’s disease as well as strategies for providing care, and tips for managing caregiver stress.
Access Options offers the following consultation packages, or we will customize a package based on individual needs:
Cooking, cleaning, grocery shopping, managing medications…we often take for granted our ability to do these things, until we are faced with challenges. There are many community services available to support seniors in their homes; however navigating these options can be complex. The standard ADL ASSESSMENT package consists of two visits with an experienced Occupational Therapist with the option for additional follow-up visits if requested. The therapist will visit with you in your home to complete an assessment of your needs, and will follow up with detailed information and resources to help you to manage your daily activities in your home. Our Occupational Therapists are also experienced in the assessment of cognitive abilities (e.g. memory, attention) and a can incorporate a cognitive assessment into the visit if requested. The information provided may include:
- Recommended community services and how to access them
- Information about ways to prevent falls in the home
- Strategies for living with dementia or memory loss
- Education about ways to conserve your energy, make your jobs easier and protect your joints…..all customized to your goals and daily routine
Preparing for the future and making the decision to move out of your home can be overwhelming. You may be asking…when is it the time to move? What types of alternative living options are out there? What options are in my neighbourhood or close to my family? What services will they provide? How much do they cost? You may also be considering downsizing to a smaller home but are not sure what to look for in order to meet your long term needs. Our experienced consultants will work with you to identify your goals and priorities and will offer professional guidance to help you prepare for this type of transition. The standard LIVING TRANSITIONS package consists of two visits with an Occupational Therapist with the option for additional follow up visits if requested. The information provided may include:
- Recommended level of care for you
- Things to consider for your future care needs
- Detailed list of options for living environments in your desired area
- What to look for when downsizing to ensure optimal accessibility
